Interfolio/教师 180 is used to file your 学院年报 (FAR) and your 利益冲突/承诺冲突 (CoI/CoC). Interfolio应该通过Agora访问. 进入Agora后，点击“我的服务”下的“学院年度报告”链接."
院系年报 and 利益冲突/承诺冲突 disclosures are due once annually, however you are encouraged to update your information continuously throughout the year. 截止日期因学校而异. Guidance for Completing your 承诺冲突/Conflict of Interest Disclosures
教师 members are required to complete Conflict of Commitment/Conflict of Interest disclosures annually. 所有教员必须填写以下表格:

承诺冲突 occur when a faculty member’s commitment to external activities adversely affect his/her capacity to meet University Responsibilities.

You will receive an email each year from the 教务长’s Office notifying you that your reports are ready to be completed. 一旦你收到邮件, you can find your annual report and conflicts disclosure 形式 in two places:

This error message indicates that you have previously reported activities that require review. 当你看着你的表单, 任何有红色“活动需要你审查”标志的部分, 需要被审查吗.
Any activity that was reported with an end semester of “ongoing” will require annual review until you indicate that activity has ended. 检讨正在进行的活动, 单击“正在进行的活动”或“活动结束”的单选按钮.” If you select “Activity Ended” you will be asked to provide some additional information. 当您检查了所有正在进行的活动后，单击底部的UPDATE.

是的! There are two places from which you can print and save a copy of your disclosures.

Select 学院年报 from the drop down and select the appropriate years for the reporting period (this year for example will be Start: Spring 2020 and End: Fall 2020). 点击 打印 & 预览 下载PDF格式的年度信息披露文件. This PDF will include your annual report and all of your conflicts disclosures in one document.
2. 您也可以通过以下网站下载报告的打印版本 Vitas和biosketch 在左侧导航栏. Once on the Vitas和biosketch page, click on the eye icon to the right of 学院年报.
进入简历查看页面后，选择 学院年报 from the drop down and select the appropriate years for the reporting period you wish to print. 导出/共享并选择PDF下载报告.

Once in the appropriate form, click on the relevant section and find the entry that you need to edit. 点击 the pencil icon at the right side of the entry line to make edits to the entry. 请务必电子游戏正规平台您的披露日期并点击保存.
Please go back into Interfolio to double check that your report or disclosures were actually submitted. 如果您的主页上仍然有任何表单，则说明它们尚未提交.
